In Palm Desktop choose Export from the File Menu. The dialog box that comes up has three choices with pull down menus. For "Module" choose Addresses. "Items" includes the addresses you have chosen in your list--you should either clean up your list first or just choose all the addresses and clean them up in Apple's Address Book. For "Format" choose V Card. Decide where you want the file saved and then click Export. Now open Apple's Address Book and under the File Menu choose Import and then V Card. That should import all of your contacts. The names and phone numbers should be fine. The messy part is that if you have made special fields on your Palm contacts, they will probably be placed in the wrong fields in Address Book. That means you might want to spend the time to manually clean them up. Now sync your iPhone and all Palm contacts should now appear.
This wasn't as difficult as you might think. Although, it was frustrating at first because I couldn't find much consistent info on forums. When we updated the sync between the palm treo and the PC, we had it sync with the Palm tool. I selected the contacts, and did an export to .vcf file type. I then opened up Outlook Express and imported the .vcf file, and contacts were visible. If you don't have iTunes on your PC, or it is outdated, you must install the latest version of iTunes. Next, connect your iPhone to your PC via the USB cable provided. iTunes open automatically. Once you register the iPhone with Apple, it will auto-sync between iPhone and iTunes for you. Quite easy, once you've done it. Hope this helps
Connect your iPhone to your PC, open iTunes and click on the device. Go to the Info Tab and choose the service that you want to use to synchronize your contacts with. Contacts that are stored on your PC in e.g. vcard format can be imported to Windows Contacts and synchronized over iTunes. Otherwise you'll have to use 3rd party apps, that let you transfer contacts directly from the PC to the iPhone.

Get the Palm program "File Man", or other advanced file program. Using this program, go to the "RAM" tab and find "Contactsdb.pdb". copy this to your SD card. Now, if you want to restore your contacts, just copy it back (you will need to hit the Replace Anyway button).
